Specimen Requirements

A peak level should be drawn 30 minutes after completion of infusion of an intravenous dose or 60 minutes after an intramuscular or oral dose of vancomycin.

Note:  Label specimen appropriately (peak).

Container/Tube:  Lithium-heparin (light green top) or Red Top

Preferred: Lithium heparin (light green-top)    *****NO Serum Gel (SST)*****

Draw blood, spin down and send 1 mL (pediatric:  0.2 mL) of plasma or serum.
